Call of Duty Modern Warfare 3 is a direct sequel to the previous game in the series, Call of Duty: Modern Warfare 2, continuing the intense narrative of the original Modern Warfare trilogy. The game's single-player campaign follows Task Force 141, led by Captain John "Soap" MacTavish and Captain John Price, as they hunt the Russian Ultranationalist terrorist Vladimir Makarov, who is orchestrating a large-scale war across Europe and the United States. Players will engage enemy forces in iconic global locations such as New York, Paris, and Berlin, facing off against Makarov's forces in a desperate fight to prevent civilization from collapsing. The campaign features a dynamic structure divided into three acts, with missions that require players to complete various objectives, from eliminating enemies to planting explosives on enemy installations.

The game's multiplayer mode introduces significant changes, most notably the replacement of the Killstreak system with a new Pointstreak system, where points are earned not just through kills but also by completing objectives like planting bombs or capturing flags. This system is organized into three distinct "strike packages" – Assault, Support, and Specialist – offering diverse rewards based on player performance. Modern Warfare 3 also features a revamped ranking and unlock system, a new Prestige Shop for players who have prestige, and the return of the Special Ops challenge mode with more demanding missions. The game includes several new game modes, such as Kill Confirmed, where players must collect dog tags from fallen enemies, and the innovative Survival mode, which allows one or two players to fight through increasingly difficult waves of enemies.
