In the year 2037, a destructive plague has ravaged the streets of a large metropolitan city, and a radical bio-terrorist organization has taken a team of scientists hostage, threatening to unleash further devastation. As the body count rises, you assume the role of John G. Ash, the leader of an elite Special Forces squad known as  X-Squad, tasked with eliminating the terrorists, rescuing the scientists, and saving the city from further destruction. The game features 3D gameplay where you command a four-person squad through diverse environments, including subway systems, sewers, and city streets, utilizing the PlayStation 2's graphical capabilities to render effects like rain, fog, smoke, and mist.

You lead your team of specialists—Maya, Melinda, and Judd—each possessing unique features and special abilities, such as demolitions, sniping, and close combat. Using artificial intelligence, you assign orders and objectives to your squad members, who can help each other out through a buddy system, though their intelligence is limited and they often act as decoys rather than strategic assets. The game's control scheme makes effective use of the PS2's DualShock 2 controller, with the left analog stick for movement and the right stick for aiming and turning, allowing for precise control during intense firefights. You are equipped with a massive arsenal of weapons, including handguns, automatic rifles, sniper rifles, rocket launchers, and grenades, to combat enemies who infiltrate from the sewers and rooftops.
